Why Should I Have My Wisdom Teeth Removed?

Not all impacted wisdom teeth necessarily need to be removed.  However they are indicated for extraction in certain conditions like:

  • Infection
  • Pathology (cysts, tumors)
  • Risk of damage to adjacent teeth
  • Chronic pain or discomfort. ,
  • Crowding of teeth
  • Unpleasant taste or bad breath in the mouth

When is the right time to get my wisdom tooth removed ?

There are many factors which determine the removal of wisdom teeth like age, symptoms (Pain, Infection, Swelling, Bad breath, difficulty in opening mouth) prophylactic removal etc. However our Maxillofacial surgeon, Dr Ranjit Kumar will examine you and weigh the risk versus benefits of removal of wisdom teeth and advise you appropriate treatment.

What type of dentist performs wisdom teeth removal?

Ideally wisdom tooth removal is best done by an Oral & Maxillofacial surgeon as they are well trained & equipped in performing these procedures.

What type of anesthesia is used?

Depending on the difficulty of wisdom tooth removal number of options of the type of anesthesia  are available

  • Local anesthetic: The Gum surrounding the wisdom tooth is injected with Local anesthesia. While the wisdom tooth is removed you will feel no pain as the area surrounding the tooth is numbed in a couple of minutes. For wisdom teeth which are simple to remove it is the best option
  • Local anesthetic and Inhalational sedation: Nitrous Oxide Inhalational anesthesia is advised in dental patients who have anxiety, low pain tolerance, underlying psychiatric disorders, and mental retardation. By this, you will be less aware of the procedure, and makes you feel relaxed.
  • General anesthesia: Under a “day case” general anesthetic, it is possible to do wisdom teeth removal. You can go home on the same day of procedure, even though you are put to sleep completely at the time of procedure.
How long does it take to remove a wisdom tooth?

The time taken for the removal of wisdom teeth varies. It takes only a few minutes for some wisdom teeth. It can take around 30 minutes for more difficult wisdom teeth which need to be cut into pieces.

Is there much pain or swelling during & after the removal of wisdom teeth?

Removal of wisdom teeth is relatively a Painless procedure. However after procedure, both on the inside and outside of the mouth, there is likely to be some swelling and discomfort. It may take up to 2 weeks for all soreness to be completely eliminated. You may need to eat soft diet for a week, and may also find that your jaw is stiff.  Our Surgeon, Dr ranjit Kumar will prescribe pain killers if your jaw is sore. After extraction, it is also necessary to use a course of antibiotic for you. It takes a fortnight to completely heal.

What are the possible problems following removal of wisdom teeth?
  • There will be a little bleeding at the time of surgery, which usually stops quickly, and it is not a problem as it is stitched normally. In case if the area bleeds again after going home, it is easily controlled by applying pressure by biting on cotton or gauze cloth for 10 minutes. Please contact our office, if bleeding does not stop.
  • Tingling & numbness of lower lip & tongue is a rare occurrence when the nerve is in close proximity to the impacted tooth.
  • Restricted mouth – Due to prolonged opening of mouth or Nerve block injection

What are facial fractures?

Fractures (broken bones) of face and mouth are called facial fractures. Lower jaw (Mandible), upper jaw (Maxilla), nose (Nasal), bones surrounding eyes (Orbit), and cheekbones (Zygoma) are the commonly included facial fractures.

If you have been unconscious, or if you have any fractures in the face, our Oral and Maxillofacial Team thoroughly check for any other injuries or fractures of head and neck and make sure that you don’t have any other concomitant fractures.

What causes facial fractures?

The most common cause of facial fractures is Road traffic accidents. The other causes include inter personnel violence, sports injuries, accidental fall etc. Sometimes dental cysts & tumors can cause weakening of bones and might result in fracture of the jaw.

How will I know if I have a facial fracture?

Your facial and any other fractures are checked by our Maxillofacial Surgeon, Dr Ranjit Kumar if your face has any kind of sharp or blunt trauma. Our Surgeon will study your x-rays and also carefully examine you for any pain or swelling or for any changes in mobility. In dental, head, jaw and facial x-rays most fractures are visible clearly as dark lines. Further x-rays & Scans and follow-ups are required for some hairline fractures as they are very difficult to see.

What are treatments for facial fracture?

Upon examining you, our Surgeon will advise you an appropriate line of treatment which might be either a) Conservative method Or b) Surgical method of treatment.

When the fractures are undisplaced & occlusion is norma,l usually Conservative methods are advised which include Intermaxillary fixation(IMF) wherein the jaws are immobilized. When the injuries are more grievous, avulsive & displaced, surgical mode of treatment is advised where Open Reduction & Internal Fixation (ORIF) is advised. More explanation about your fractures and their treatment will be given by our Oral and Maxillofacial Surgeon.

I have a facial fracture, how long will it take to heal?

Based on the treatment you had , extent of fracture & your general medical status the time required for healing of your facial fracture depends. It takes 2-3 weeks for swelling and bruising to completely settle. Your look begins to appear normal at this point of time. However healing continues inside after this time. You have to wait and be patient till it is healed completely.

My face is numb or feels odd, after a facial injury, how long will this take to heal?

Small nerves in facial bones run in many channels. Sensory nerves which help in sensing pain, hot, touch, and cold pass through these bones. The skin of the face and soft tissues are supplied by these nerves which distribute themselves after emerging out of bones.

Altered sensation of face / numbness and short-term pain may be seen when you have facial injuries. For this altered sensation to disappear completely, it takes a number of weeks or even months. No recovery or partial recovery occurs very occasionally.

What are plates used to treat facial fractures?

Stainless steel miniplates ,Titanium miniplates & Biodegradable/Resorbable plates are  used to repair facial fractures. However Titanium miniplates are widely used as they have the advantages of being more bio-compatible, rigidity & ease of application,  Biodegradable/Resorbable plates are also being used for children .

Will I have to have the plates removed?

The Stainless steel & Titanium miniplates are generally left behind & not removed unless they are infected which occurs very occasionally.  The Biodegradable/Resorbable plates  resorb over a period of time.

Will the plates set off metal detectors at airports?

At airports, the plates do not set off metal detectors as they are made up of titanium.

Do I need to tell anyone that I have a metal plate in my face/jaws?

While having computerized tomography (CT) or magnetic resonance imaging (MRI) it is wise to inform them beforehand about the treatment procedures you have undergone for facial injuries.

Why have I been told that I shouldn’t blow my nose for the next few days?

For about ten days after injury, you should not blow your nose, if you have fracture in your cheek bone. Because it may result in facial swelling around the eyes, as you might force air from your sinuses.
This swelling disappears with time as it is temporary. Our Oral and Maxillofacial Surgeon will advise you if you have any queries.

If I have had a facial fracture, what other things should I avoid for the next few weeks?

The underlying instructions should be followed which are advised by our Surgeon

  • To avoid hard food
  • To not to blow your nose
  • To avoid any active sports
  • To maintain good oral hygiene.
  • To take rest for a couple of weeks.
